Authorization
​By signing below, you authorize Billings Jiu Jitsu, LLC to initiate recurring Automated Clearing House (ACH) debit entries from the bank account provided for payment of membership tuition and/or selected services.
Billing Terms

Recurring membership tuition is billed monthly on the 3rd day of each month, unless otherwise specified in writing.

Charges will appear on your bank statement as an ACH debit from Billings Jiu Jitsu, LLC.

This authorization remains in effect until properly canceled in accordance with the cancellation policy below.
Attendance & Usage
Membership billing is not based on attendance or usage. Failure to attend classes, temporary absence, or non-use of facilities does not suspend, pause, or cancel billing.
Cancellation Policy

Cancellation must be submitted in writing by the account holder.

Written cancellation notice must be received at least fourteen (14) days prior to the next scheduled billing date.

Acceptable written notice is limited to email sent to: info@billingsjiujitsu.com.

Verbal requests, text messages, social media messages, or failure to attend classes do not constitute cancellation.
Refund Policy
Charges processed in accordance with this authorization and cancellation policy are non-refundable. Refunds or credits, if any, are issued solely at the discretion of Billings Jiu Jitsu, LLC and do not constitute a waiver of policy.
Authorization & Revocation
You certify that you are an authorized signer on the bank account provided. You may revoke this ACH authorization only by submitting written cancellation notice in compliance with the cancellation policy above. Revoking ACH authorization without proper cancellation of membership does not eliminate amounts owed under this agreement.
Disputes
You agree not to initiate a bank dispute or chargeback for transactions processed in accordance with this authorization. Unauthorized disputes may result in immediate suspension or termination of membership privileges.
Acknowledgment
By signing below, you acknowledge that you have read, understand, and agree to the terms of this ACH authorization and membership billing policy.
